This Doctor's office refused to answer some basic questions about insurance and treatment locations or availability until I completed an application requesting personal information. Very strange! None of the questions I was asking was in reference to actual care or diagnosing medical conditions, the office then sent me a nasty email saying that I should look for care elsewhere since I refused to follow their instructions.
