I had Dish network installed back in December of 2018 and used promo code CARD150 through GoDish. Several months later I inquired about my card because I hadn't received it, at that time I was informed that I needed to fill out a rebate form to receive my card. In March 20 2019 I called godish.com and had it installed 3/21/19. The representative I spoke with asked me for the promo code I saw on Facebook, I gave him card100. He said great, and that I will receive my giftcard in 60 days. I called today to find out the status of my giftcard, the rep today said they didn’t receive my rebate form. I said what rebate form?
