Went downhill this surgery has, cant get appointments and they never follow up for appointments when tested etc.
I have provided all information of my family through filling registration forms, contacting few time in period of 4 months they did not put information in record. They were not able to register my four daughters and me(Father). And the worst thing they were aware of submitted froms and registration.
