Ruined my life. Clinician's poor note taking led to factually incorrect information being noted on my permanent record; no-one willing to accept responsibility for making a mistake. Keep on saying it is their "Duty of Care" to uphold lies. I was constantly passed on by different members of staff - shut down by each of them. A service which is meant to help mental health has caused me nothing but harm. I am writing to express my deep disappointment with Mind Matters mental health service in Surrey. Last year, during a very dark period in my life, I reached out to them for help. It is now June 2024, and after contacting them in September of last year, it took them approximately nine months to respond to me. I've been in need of help for months. Been suicidal and really struggling. Self harm etc and was told they'd get me an appointment within a week or 2 . Its now been four months of me and my partner chasing them and still no help and constantly being fobbed off. I am not being helped which is making everything worse. Let's hope I make it through another week alive bear in mind I have kids too .... so wrong
