This was at the valencia location. I had to go to work and asked my mom if she could take my daughter to get checked. Never had I ever hear that you need a NOTARIZED letter. Are you serious?? Not even at the doctors they asked for a Notarized letter. My mom said the ladies Were very rude.

Dr in Maricopa was terrible. Wouldn’t listen to me, rushed through exam, told me I was fine to drive with my eyes dilated-I wasn’t, gave me a prescription I can’t see out of, charged me for a contact lens fitting that my insurance covers, and now I have to pay for an exam someplace else since my insurance only covers one per year. My advice-stay away from this location.
