Was it clear that this provider takes pride in the appearance of his/her office? Yes, the office was well organized and taken care of Did you have to wait long after you arrived for your appointment? No, I didn't have to wait very long Is this provider's business conveniently located? Yes, it is very conveniently located

Was there annoying music playing while you waited or during your visit? No, there was no annoying music Was it easy to find parking at this provider's office? Extremely easy, and close to the entrance Did this provider promise services he/she couldn't provide? Not at all! They exceeded my expectations Does this provider always take that extra step to make you feel special? Absolutely, they always make me feel like I'm their only patient.

Was this provider's office too loud? Not at all, it was very peaceful and quiet Did this provider use excessive foul language? Not at all! They are always respectful and courteous! Were you able to get answers to simple questions from this provider without scheduling an appointment? Yes

Was this provider willing to stay open late to accomodate your needs? Definitely! They would never turn away anyone in need How would you compare this provider to others in his/her field that you have visited? There is no comparison! Did this provider pressure you to purchase any unnecessary products during your visit? Absolutely not! I was never uncomfortable

Was this provider's office clean? Yes! I was amazed at how clean it was! Did this provider seem well-trained and experienced? Yes, they seemed very qualified Did this provider have an entertaining selection of waiting room magazines? Yes
