Did you experience unnecessary pain during your visit? No Did a staff member acknowledge or greet you when you entered? Yes, I was greeted when I walked in Was it easy to find parking at this provider's office? Yes, it was convenient Did this optometrist seem knowledgeable about different types of contacts? Sort of, I wasn't entirely convinced she/he knew what they were talking about Did you have to wait long after you arrived for your appointment? I had to wait for a little while.

Did this provider seem up-to-date with the current advancements in their field? Yes Did this provider wear an obnoxious cologne? No, they didn't wear any cologne Did this provider have an entertaining selection of waiting room magazines? Yes

Yes Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me Was this provider late to your appointments? No, they were on time Does this provider offer any coupons, discounts or promotions? Not that I have seen

Did this provider ever dismiss your concerns as unimportant? No, they take every concern of mine seriously Did you spend a lot of time in the waiting room at this provider's office? No, my appointment started at the scheduled time Was this provider argumentative or easily angered? No, they were very calm and understanding Did you feel safe in this provider's care? Yes Does this provider remember you and your circumstances at every appointment? Yes

Was this provider's office clean? Yes! I was amazed at how clean it was! Does this provider use current and safe equipment? Yes; the equipment is very advanced! Was the waiting area spacious? Yes Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me
