It is important to note that you are asking for a review after the initial consult, when the patient has not even had the test for which she is seeing the doctor. There becomes a time the surveys to find out if you did a good job are merely irritating your target audience. I'm surprised that your marketing company does not realize this has become detrimental to the practice's reputation. Please ask them to stop.
