Last visit on January 31, 2019, was for a periodontal evaluation. The visit went well. Front Office had me pay $16.70 as my share. Delta Dental sent me a notice saying the amount submitted was $167.00 and was fully paid by the insurance carrier. How is this to be resolved?

We just had our first visit with Dr. Azizi after switching from our previous provider. I was somewhat familiar with Dr. Azizi’s practice in the past for my mother’s dental care. I was very impressed back then, so when we decided to switch from our current provider after 40 years of driving to Concord, Dr. Azizi’s name came to mind.
