Your staff failed to use pt’s two important identification. Pt is required to provide ID but your staff never checked or confirmed is pt’s identification correct. I had an US today but I still don’t even know her name. Your staff from front desk to the exam room failed to introduce their names to the pt. How hard is that??? Friendly reminder pt’s identification is one of the patient safety national goals.
