Comment is re: Q4 - seems like you think a 14 minute wait is no wait. I'd add a 1-14 minutes to your options, and then I would have checked that bubble. Also, last time I was in I asked to only receive email notifications & reminders of my appts, but this time I continued to get text and email notifications - a total of 6 (everytime I was emailed I was texted as well - one time on Saturday morning at 8am which was annoying).
