Having to complete the information sheet each year for each child seems unnecessary. This seems to be something that can be reviewed, edited, and initialed like the health history forms. I only have two children and this took me most of my wait time to look up SS#'s and DL#'s for the whole family. I know all this information was completed last summer already. With computers, you could even email the form to review or complete before we got there. It wasn't a problem, per se; however I thought it was a bit irritating to compl... read more
