Was this provider friendly? Yes Did a staff member acknowledge or greet you when you entered? Yes, I was greeted when I walked in Has this provider ever cancelled your appointment on you last minute? Never, they are very reliable. Did this provider use excessive foul language? Not at all! They are always respectful and courteous! Did you feel like this provider's office was understaffed? No, they were fully staffed

Not at all, I showed up early and they were ready to see me Did you spend a lot of time in the waiting room at this provider's office? No, my appointment started at the scheduled time Has this provider ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute Were you able to relax during your appointment? Yes

No, they take every concern of mine seriously How would you compare this provider to others in his/her field that you have visited? Among the best I have seen

Yes Did you notice a foul odor when you arrived at this provider's office? No, I didn't notice an odor Is this provider easy to reach in an emergency? Somewhat, they eventually called me back

Does this provider offer flexible appointment times? Absolutely! I never have a problem! Was it clear that this provider takes pride in the appearance of his/her office? Definitely! It was one of the best looking offices I've ever seen! Was this provider late to your appointments? No, they were on time
