Kim and Tanya were excellent as always. One very big annoyance to me is the annual patient form update. I understand asking about med changes, signature to make certain you get paid, etc; however, I find it very offensive after coming to you for 10+ years to be handed a totally blank form that assumes you do not even know my name, address, the email address you sent this survey to or my cell phone that you texted my appointment confirmation to. Thank you so much for your review! We love seeing you as a patient of our practice. Your feedback is valid and appreciated. Due to HIPAA we need to have the entire form filled out by the patient . We do have the option for our patients to update their information online prior to their appointment. We are going to further research this to see if there are any updated changes that will allow us to make this simpler.
