Were you able to relax during your appointment? Yes, I was completely at ease Does this provider adhere to the customer is always right policy? Absolutely, the customer is always right here Was this provider late to your appointments? Not at all, I showed up early and they were ready to see me Was this provider argumentative or easily angered? Not at all, they were one of the kindest, most attentive individuals I've ever met

Did this provider promise services he/she couldn't provide? No, they were able to provide everything they said they would Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me Did this provider's staff seem to respect him/her? Yes, I could tell the staff respected him/her Did you notice a foul odor when you arrived at this provider's office? No, I didn't notice an odor

Did this provider pressure you to purchase any unnecessary products during your visit? Absolutely not! I was never uncomfortable Was there annoying music playing while you waited or during your visit? No, if they had music on it was very soothing Were you able to relax during your appointment? Yes, I was completely at ease Does this provider remember you and your circumstances at every appointment? Definitely! They always remember me and make me feel important!

Yes Does this provider leave you on hold for a long time when you call? No, they are really great about answering the phone right away Was this provider's office too cold? No, it was fine
