My girlfriend scheduled for me an appointment. They called me and said I needed to come in to provide proof of insurance. I came in and realized that I had misplaced my dental card. Even though I had misplaced it, I called my provider and was able to give them the information that was on my card to provide proof of coverage. But that wasn’t good enough for them.
