I have an appointment with Dr. Lai this July 13 and 14. I was promised pre exam test instructions that would be sent to my email address before the end of this week to prepare me for the nuclear stress test ordered the previous week ago. I would appreciate the follow up asked for by the end of the day this coming Monday July 11, 2016. I will follow up with a fax and call to have this my request verified.
