Do you feel that you could have received better service somewhere else? Definitely not; I received the best service ever! Did this provider seem up-to-date with the current advancements in their field? Yes Is this provider easy to reach in an emergency? Yes, they answered my call right away Does this provider always put the customer first? Absolutely! They have some of the best customer service I have ever seen

Did this provider thoroughly explain the risks and benefits of your treatment? Yes, they made sure I had all the information I needed to make a decision Did this provider wear an obnoxious cologne? No, they didn't wear any cologne Was it easy to make an appointment with this provider? Yes! It was effortless! Was this provider argumentative or easily angered? No, they were very calm and understanding

Does this provider offer any coupons, discounts or promotions? Not that I have seen Did this provider listen to your input and concerns? Absolutely! This provider always listens carefully to what I have to say! Does this provider leave you on hold for a long time when you call? Never! I don't think I've ever been put on hold here for a long time

Was this provider willing to stay open late to accomodate your needs? Yes, they've stayed open late when I've needed help Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! Did you leave the office feeling satisfied with your visit? Completely!

Did you feel like this provider's office was understaffed? Not at all, they had plenty of staff members to help me whenever I needed assistance Did you feel safe in this provider's care? Absolutely! I knew I was being cared for by an expert! Would you refer this provider to a family or friend? Yes, I'm going tell everyone about them. I absolutely love this place! Does this provider treat every client equally? Definitely, they make everyone feel equally respected and cared for

Has this provider ever cancelled your appointment on you last minute? Never, they are very reliable. Was the waiting area spacious? Yes Did this provider listen to your input and concerns? Absolutely! This provider always listens carefully to what I have to say! Did the staff make you feel uncomfortable when you called with questions or concerns? Not at all! They were happy to address every concern I had

Yes, it never fails. I'm put on hold forever when I call Did this provider pressure you to purchase any unnecessary products during your visit? No, my decisions were always respected

Was it clear that this provider takes pride in the appearance of his/her office? Yes, the office was well organized and taken care of Were the staff members able to answer your questions? Yes, they were able to answer my questions Did this provider ever dismiss your concerns as unimportant? Never, they go out of their way to address every concern of mine immediately

Does this provider treat their staff well? Very! I've never seen an employer treat their staff better Did you experience unnecessary pain during your visit? A little, but it was bearable Does this provider offer any coupons, discounts or promotions? Not that I have seen Was this provider's office too cold? No, it was fine
