Do you feel that you could have received better service somewhere else? Yes, I think I could have gotten better service elsewhere Did you leave the office feeling satisfied with your visit? Not satisfied, but not unhappy Does this provider treat every client equally? Yes, they don't show favoritism Did this provider wear an obnoxious cologne? No, they didn't wear any cologne Did you spend a lot of time in the waiting room at this provider's office? The wait wasn't terrible, but it wasn't the best

Was this provider's office easy to locate? Absolutely! It couldn't have been easier! Did this provider wear an obnoxious cologne? Not at all, their cologne smelled great Was this provider late to your appointments? No, they were on time Were you able to get answers to simple questions from this provider without scheduling an appointment? Absolutely! I'm always able to get my questions answered over the phone right away! Did this provider leave you unattended for an extended period of time? No, I was always attended to

Does this provider treat every client equally? Definitely, they make everyone feel equally respected and cared for Does this provider welcome questions? Absolutely! They are happy to answer all of my questions Was this provider sympathetic to your situation? Yes! They went above and beyond to make me feel special and respected Was this provider's office too cold? No, it was fine Did you leave the office feeling satisfied with your visit? Completely!

Was this provider's office too loud? Not at all, it was very peaceful and quiet Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed Was this provider willing to stay open late to accomodate your needs? Yes, they've stayed open late when I've needed help Does this provider welcome questions? Absolutely! They are happy to answer all of my questions Was this provider's staff friendly? Yes, they went above and beyond
