Dr. Allen owns multiple surgery centers. I had cataract surgery on my right eye at one of his surgery centers and cataract surgery on my left eye at another. His office staff told me to give my $500 deductible to one of his surgery centers, but then I got a bill for my $500 deductible from his other surgery center. After 6 months, one of his surgery centers has not returned my deductible and the other is threatening to send me to a collection agency and ruin my credit rating.
