Nice doctor, did surgery on my frozen shoulder. Unfortunately, I can't say much about his staff at the summit health plex. Took them nearly a month to submit my continous fmla paperwork in, and its been over a week and still waiting for them to submit my intermittent fmla paperwork for my physical therapy. I've called the office, leave a message and nobody ever contacts me back.
