Did this provider wear an obnoxious cologne? No, they didn't wear any cologne Was the toothpaste used during your cleaning enjoyable? Yes Did this provider seem well-trained and experienced? Yes, they seemed very qualified Did the staff make you feel uncomfortable when you called with questions or concerns? Not at all! They were happy to address every concern I had

Were you able to relax during your appointment? Yes Have the fillings, crowns, or veneers you've received lasted? Not at all, they fell off almost as soon as I walked out the door Was it clear that this provider takes pride in the appearance of his/her office? No, I don't think they've ever cleaned it Was this provider's office clean? It was satisfactory Did this provider seem well-trained and experienced? Yes, they seemed very qualified

Did this dentist answer all of your questions? Some were answered Did this provider's staff seem to respect him/her? Yes, I could tell the staff respected him/her How would you compare this provider to others in his/her field that you have visited? Not the worst, but certainly not the best Did you feel safe in this provider's care? Mostly, I was still a little nervous Do you trust this dentist to only recommend dental procedures you need? Not really, they've recommended a few things I don't need

Did this dentist offer you anesthetic when appropriate? Definitely! They paused every few minutes to make sure I was comfortable Did this provider thoroughly explain the risks and benefits of your treatment? Yes, they made sure I had all the information I needed to make a decision Did this provider promise services he/she couldn't provide? Yes! The waiting room here is spacious. I never feel like I have to sit too close to someone or that it is overcrowded. They always take the time to listen to me. I know they really care about me and what I have to say. Their office was nice and quiet. Sometimes the receptionists are rude, however. If I didn't really, really like my dentist (Dr. Williams), I would probably go somewhere else because of the receptionists. One time I sat there for a half hour, and no one called on me. Did this provider have reasonable prices? No, it was expensive. Was this provider's staff friendly? Friendly Enough Does this provider always put the customer first? Somewhat, but I didn't feel like they cared about my needs Did this dentist practice good oral hygiene? Yes, I could tell they practiced good oral hygiene Was it easy to make an appointment with this provider? Practically Impossible

Yes Does this provider maintain a professional appearance? Somewhat, I wasn't overly impressed Does this provider welcome questions? Yes, they don't mind answering my questions
