I can't thank our hospice team enough. The care and support from Community made a significant impact on my dad's journey, ensuring he was treated with dignity, respect, and compassion. I have been familiar with this excellent organization for many years. Three months ago, I put my stepmother in a nursing home, where she receives hospice care from CHOT. I can't praise Joan's care team highly enough - all are caring, competent and compassionate. The social worker Taryn and bathing lady Martha are both johnny-on-the-spot. My primary contact, Anna, goes way above and beyond in her consistent, thorough communications. Their support makes a big difference - every day.
Helped our family through a very difficult time with compassion and professionalism. Very responsive when help was needed and I would highly recommend them.
I used commiunity hospice for my husband and they were so great.. really felt safe with them coming and helping me care for my husband.
I would like to thank your nurses and CNA’s for your service while my hisband was at Stonegate nursing and rehabilitation.. I do not recommend this facility to any patient that transitioning they give poor service not only to the patient that’s Transitioning but their regular patients.. I rate them a 0.. very, very poor..
