I really hate the process for getting forms filled out. My daughter needs her physical forms today. I dropped off last Tuesday. I can understand 48 hours but now we are in another week and my daughter can't practice today without the forms!!!

Because our providers travel between offices, our goal is to have any forms dropped off to be completed within one week. Many times they can be completed sooner than that.
