The dental work has always been first rate, but please eliminate having to fill out the same form every few years. Why can’t you print out the previous answers and have the patient change the items that need changing?

My family have been patients at Caulks Hill Dental for at least 20 years. I just visited for the first time in 14 years (Didn't visit any other dentists.
