Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me Did this provider's staff seem to respect him/her? Yes, I could tell the staff respected him/her Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! How would you compare this provider to others in his/her field that you have visited? Among the best I have seen Did this provider listen to your input and concerns? Yes

No, I was in a little pain but it wasn't excessive Was the water they used during your teeth cleaning a comfortable temperature? Yes, it was fine Would you refer this provider to a family or friend? Yes Did you experience unnecessary pain during your visit? No

I always get sound, helpful, and completely unbiased advice! Did this provider ever postpone your appointment? Not at all! They always try to get me in as soon as possible Was this provider sympathetic to your situation? Yes! They went above and beyond to make me feel special and respected
