RecommendedBeautiful place. Wonderful staff. Been twice to do Cool Sculpting with Jordan - she is so sweet, capable and “on it”. Dr. Ward is caring, sincere and overall a great guy. Nurse SanDee is top shelf as well, always attentive and communicative. 5 stars!

The staff is amazing and the facility is fantastic. More importantly, Dr. Ward is very knowledgeable and up-to-date on procedures. His advice was honest and specific to me. I didn't feel like a number; I was someone he cared about helping.
I highly recommend Dr Ward as well as all his staff at Form Spa. I struggled with serious acne during my pregnancy that left me with unwanted scars. Dr Ward did a Co2 procedure on my face and I am very happy with the results. The staff was welcoming and friendly and the facility is beautiful.
