Did this provider leave you unattended for an extended period of time? No, I was always attended to Does this provider welcome questions? Absolutely! They are happy to answer all of my questions Did this provider ever dismiss your concerns as unimportant? Never, they go out of their way to address every concern of mine immediately Was this provider's office too cold? No, it was fine Were you able to relax during your appointment? Yes, I was completely at ease

Was this provider's staff friendly? Yes Was this provider's office easy to locate? Absolutely! It couldn't have been easier! Was it easy to make an appointment with this provider? Yes Did this provider's staff seem to respect him/her? Yes, I could tell the staff respected him/her Did they follow up with you after your appointment? I had a follow-up appointment, but I scheduled it.

Did this provider leave you unattended for an extended period of time? Not at all! They were always present and attentive Did this provider rush your appointment? No Does this provider leave you on hold for a long time when you call? No, they are really great about answering the phone right away Did this provider seem irritated to be working with you? No, they were polite Did this provider seem well-trained and experienced? Yes, they seemed very qualified
