I was a patient at UAB Gardendale. I am for a form to be filled out which I dropped off a month ago and I received every excuse why they was not going to fill it out. First, I needed to be seen in a clinic, which I was. Then, I need to schedule an appointment. NOT. My insurance do not pay for the same double care.
