Filled out the forms online before my appointment as requested and waited for 30-35 minutes in the lobby before the receptionist said they couldn’t find my forms and asked me to fill it out it again. Additionally, they made me pay for contact lenses services that have not been provided yet.
The doctor was great, the people at the desk were not.
