Arrived there from the hospital on a Friday at 1030 am for physical therapy rehab. Told It wouldn't start until the following Monday at which time I would see a physical therapist. Spoke to a director of nursing. Told her that was unacceptable and asked to be transferred out of the facility. She had a physical therapy assistant see me then. Interesting that what they mean by seeing a physical therapy on Monday was actually a 10 min Video Visit. Substandard! This video visit was the only time I had contact with a physica... read more

My father was a patient at this facility. He was treated very well. The facility is clean, and smells fresh as soon as you enter. The lady at sign in is very nice. My father and his room were always clean and he was also. The entire staff I encountered were pleasant and communicated well, informing us of any issues. Occupational and physical therapist worked with him every day. Overall, the entire staff were great. I cannot remember everyone responsible for my dad's care. However, Whitney was a great nurse. My wife entered Autumn Care in Shallotte N.C. sick. weak and in need of nourishment & physical therapy. She became strong & self sufficient in a little over 2 weeks. She was fed in a way that was so healthy & attractive that she is able to handle most all her personal needs. She was treated with respect & loving care. Any one needing rehabilitation would do well trusting Autumn Care in Shallotte N.C. for their recovery.

I Have been gone from this horrible facility for about two weeks and am feeling so much better. i have never been treated so horrible that I still have nightmares. The night I arrived I was awakened by someone coming to take blood. She wrapped the band on my arm and instead of looking for a vein she went straight for the hand. I would give this facility a 0 star rating if I could. I have waited over a year to write a review, time to try and cool off a bit. My Father was in this facility for less than two weeks, he was recovering from a stroke and ended up passing. I found the staff uncaring, big talk, plenty of inaction. I asked many times to be sure my Father was hydrated, even posted signs in his room requesting that be done and also notified management. This was during Covid so visiting hours were limited. I recognize that many care facilities are understaffed. That said, a facility should not take on more patients than it has staff to care for them. My mother was admitted for rehab with a healing wound. Two weeks later we discharged her back to the hospital because the wound was infected due to not receiving proper or consistent care. Some staff were exemplary and took care of her as if she were their own family. Others left her sitting unattended and unclean for hours. My husband has been here 2 weeks and he has not had any care at all I have not had any feedback from a nurse doctor social worker therapist or anyone in the two weeks that he's been there not one person have I talked to at the facility about him nobody's called me texted me or emailed me what I did get told that he was being discharged that somebody did communicate that no one answered the phone at the nurses station when I called to find out and check on him I will be there to come to your the room and talk to you and never... read more
