Has this provider ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute Did this provider rush your appointment? No Does this provider ever overbook appointments here? Sometimes, they should allow for more time in between appointments Was the dental equipment clean? Absolutely! It looked brand new Did this dentist give you your invoice promptly? Not really, it took a long time

Did this provider leave you unattended for an extended period of time? No, I was always attended to Have the fillings, crowns, or veneers you've received lasted? Yes, they lasted as long as they were supposed to Did this dentist practice good oral hygiene? Definitely! They had one of the brightest smiles I've ever seen! Did you experience unnecessary pain during your visit? No Did this provider listen to your input and concerns? Yes

Did a staff member acknowledge or greet you when you entered? Yes, I was greeted when I walked in Does this provider remember you and your circumstances at every appointment? No. I have to re-explain my circumstances every time Did you spend a lot of time in the waiting room at this provider's office? The wait wasn't terrible, but it wasn't the best Are you confident that this provider will continue working with you until a solution is reached? Yes Did this dentist offer you anesthetic when appropriate? Yes, they offered me anesthetic when appropriate Did this dentist thoroughly examine your teeth? Yes, they examined my teeth thoroughly

These people are crooks! They wanted to charge me extra for my cleaning becaus of 'excessive' tartar. According to them, this would require me to come in for not one, but three cleanings!! I've never heard of such a thing. On top of that, they also told me I had a cavity, which a subsequent dentist said did not exist. I would have had to shell out about $160 for all that work I didn't need. The hygenist at the dentist I went to for a second opinion cleaned my teeth in 20 minutes. Excessive tartar, huh? Did they follow up with you after your appointment? Yes, they made sure I had everything I needed! Did this provider prominently display their diplomas or business license? Yes Did your mouth feel clean after your appointment? Yes, my mouth felt clean after my appointment

Are you going to visit this provider again? Not if my life depended on it Was this provider sympathetic to your situation? Not at all. It was obvious they didn't care about me Would you refer this provider to a family or friend? No, If I had known how terrible they were I never would have gone.

Did this provider ever take personal phone calls while in the appointment room with you? He/she would never do that! Their full attention is always on me.
