Apparently before my appointment I was to receive a packet to fill out. I did not receive this packet in my email, on the portal, or via mail. Everyone makes mistakes so maybe they didn’t send it??? I was a patient of Dr. Grant's for a short time then transfered my care to another psychiatrist. I'm disabled, unable to work and I'm working on my social security disability. At this time I have no income and I'm at high risk of becoming homeless. After requesting my medical records I was told I'd have to pay $25.
