I had filed out the history form ahead of time, but when I arrived I was asked to answer the questions and then sign. I made no issue of this, but familiarity with the form would allow asking whether the patient had already answered the questions and therefore only needed to sign the document electronically.
Very happy with the quick procedure and comfortable results
No issues. I like my hygienist. She's very accommodating. Also, I like being able to see my x-rays on the big screen! I think it is a good idea that the hygienist makes the next appointment while I'm still in the dental chair.
The entire staff is welcoming and friendly, while maintaining professional demeanor.

Dr Swenson, and his staff are amazing!!! I would recommend this dentist office to everyone, and I do!!!!!! We will NEVER switch offices!
Everyone at the office was very friendly and helpful. Dr. Swenson was kind and helped explain everything in a very easy to understand fashion. I will definitely continue to bring my son there in the future.
