I went in got annual physical they coded this incorrect so I do not get my wellness incentive thru my job. I called several times and no one calls back, Dee at front desk told me Dr Keizer changed his coding but apparently he did no. I went there in person to complain and get something on their letterhead stating I had my annual physical but they would not do it. They do not practice good customer service.
