Did this provider answer all of your questions? Every question I had was answered thoroughly Was this provider's staff friendly? Yes, they went above and beyond Did you leave the office feeling satisfied with your visit? Completely!

Did you notice a foul odor when you arrived at this provider's office? No, I didn't notice an odor Did this dentist provide helpful oral health techniques you could practice at home? Not really, I was given a couple tips but not very many Did the staff call you to remind you about your upcoming appointment? Yes, they called me a few days before my appointment

Did this provider have reasonable prices? Yes Does this provider offer any coupons, discounts or promotions? Not that I have seen Was it easy to make an appointment with this provider? Yes Did you experience unnecessary pain during your visit? A little, but it was bearable Did your gums bleed excessively after your appointment? No, they didn't bleed excessively

Did this provider have an entertaining selection of waiting room magazines? Yes Does this provider always take that extra step to make you feel special? Absolutely, they always make me feel like I'm their only patient. Is this provider easy to reach in an emergency? Yes, they answered my call right away Were the restrooms well-stocked at this provider's office? Yes, they had everything you could possibly need! Did you receive sample toothbrushes or toothpaste to take home? A couple, but not very many

Were you able to relax during your appointment? Yes, I was completely at ease Were you provided with payment plan and fee information before your dental treatment was scheduled? Yes, I was given all the information before my treatment Did a staff member acknowledge or greet you when you entered? Immediately! They gave me a friendly greeting as soon as I walked in

They were decent, but not very affordable Did this dentist thoroughly examine your teeth? Yes, they examined my teeth thoroughly
