Beware of using this provider! This was a very deceptive process to get a pain injection. The location is labeled as "integrative pain institute" but the "10 minute" procedure I had done ended up billing my insurance for 18k OUT OF NETWORK from "plastic surgery center" which apparently owns the facility. No one bothers to explain this or offers to use one of their in network surgery centers instead. They sign you up and you're left thinking it's covered.
