Plan is much more expensive than competing plans and reimbursement is next to nothing. Customer Service (Eric) will promise call backs, as will others, but it never happens. High price for a poor return and terrible service. Look into competing plans (4 options out there) before signing up with the Humana Loyalty PPO. It will save you the trouble of cancelling...which I just did.
