How would you compare this provider to others in his/her field that you have visited? Among the best I have seen Does this provider ever overbook appointments here? Sometimes, they should allow for more time in between appointments Was this provider's staff friendly? Friendly Enough Did you spend a lot of time in the waiting room at this provider's office? The wait wasn't terrible, but it wasn't the best Did this provider ever dismiss your concerns as unimportant? No, they take every concern of mine seriously

Did the staff make you feel uncomfortable when you called with questions or concerns? Somewhat, they should have been more polite Were the restrooms well-stocked at this provider's office? Sometimes, but most of the time they aren't Did this provider wear an obnoxious cologne? No, they didn't wear any cologne Do you feel that you could have received better service somewhere else? Probably, I wasn't impressed

Are you confident that this provider will continue working with you until a solution is reached? Yes, I know I can count on them to find a solution Did this provider listen to your input and concerns? Absolutely! This provider always listens carefully to what I have to say! Did they follow up with you after your appointment? Yes, they made sure I had everything I needed! Did this provider rush your appointment? No

Was this provider friendly? Very! The most welcoming office I've visited! Did this provider ever postpone your appointment? Not at all! They always try to get me in as soon as possible Does this provider remember you and your circumstances at every appointment? Yes Did this provider leave you unattended for an extended period of time? I was left alone for a while, but it wasn't too long Did this provider's staff seem to respect him/her? Definitely! They staff spoke very highly of him/her
