They didn't get all the root for my root canal so I had to go to specialist. Now the tooth has cracked and has to be extracted!

They charged my insurance company and me the same amount. 400$ was put on to my credit card and they got 400$ from my insurance company. They will only reimburse me with $200? I was even in the office and they came up with some kind of excuse and wouldn't pay me what I'm owed. Delta Dental says they know what they did. My bank statements do not match their records they have sent me. They will not give me FULL receipts from when I've been there. Only totals? But no explanation of charges.
