I first came to FormMD after having a Mohs procedure on my forehead and had such a great experience that I later returned to have my upper eyelids done. With no previous experience using a plastic surgeon I was referred to FormMD and Dr. Ziai, by my Dermatologist, Dr. Kim of Golden Coast Dermatology.
