It seems like a waste of time and money to ask me (a long time patient) to fill out a paper form with information that you already have on file. If the request was to make sure that I knew there was a cancelation charge, then have a small piece of paper to sign. Since the form was printed after I arrived AND the computer already had this patient demographic information, then a form with all the computer information should have been printed. In that way, I could just sign my understanding of the new policy AND not waste time ... read more
