My patient was dismissed from this practice following a toxicology screen with results consistent with prescribed medications and expected metabolites. Care was terminated abruptly, without tapering, without a transition plan, and without a referral. They only gave my patient a final prescription and naloxone. This approach disregards basic principles of safe opioid stewardship, and exposes patients to unnecessary withdrawal and harm.
