I called this office in Jan and was told I had to fill out all the paperwork first before I could make an appt. So I finally filled out all the paperwork (was asked to submit more a couple of times from the first time) and emailed the secretaries again for appt. Being a victim of ID theft, I don't like filling out personal info to a place I am not even a patient yet but I did it. I waited and waited and never got a response.
