Do you feel that you could have received better service somewhere else? Definitely not; I received the best service ever! Did you spend a lot of time in the waiting room at this provider's office? The wait wasn't terrible, but it wasn't the best Was this provider's office too cold? No, it was fine Did this provider rush your appointment? No, I was able to ask as many questions as I wanted

Was this provider argumentative or easily angered? Not at all, they were one of the kindest, most attentive individuals I've ever met Was this provider's office too loud? Not at all, it was very peaceful and quiet Did you spend a lot of time in the waiting room at this provider's office? No, my appointment started at the scheduled time

Does this provider remember you by name? No, I always have to remind them Was it clear that this provider takes pride in the appearance of his/her office? Not really, it wasn't very clean How would you compare this provider to others in his/her field that you have visited? Among the best I have seen Does this provider offer any coupons, discounts or promotions? Yes Does this provider ever overbook appointments here? Sometimes, they should allow for more time in between appointments

Completely! Does this provider leave you on hold for a long time when you call? No, they are really great about answering the phone right away

Yes, they made sure I had all the information I needed to make a decision Does this provider have helpful information available online? No, just a basic listings page Was this provider's office clean? Yes! I was amazed at how clean it was! Was it easy to find parking at this provider's office? Extremely easy, and close to the entrance

Extremely easy, and close to the entrance Are you going to visit this provider again? Without a doubt! Was this provider willing to stay open late to accomodate your needs? Definitely! They would never turn away anyone in need Has this provider ever cancelled your appointment on you last minute? Never, they are very reliable.
