I went to Crystal Smiles LLC three times. The first visit was fine (cleaning and X-rays). On the third visit, after my tooth was opened and I was given numbing shots, the nurse treated me poorly, shined the light directly into my eyes, and refused to give me sunglasses. They never submitted my paperwork to insurance so I can have my teeth fixed. No follow up from the office. When I followed up they told me it takes 2 weeks. It has been over 2 months and insurance verified they never submitted my paper work. Stay away at all cost.
