Did this provider ever dismiss your concerns as unimportant? No, they take every concern of mine seriously Was this provider late to your appointments? No, they were on time

Did this provider have reasonable prices? Yes Was it easy to find parking at this provider's office? Yes, it was convenient Was the waiting area spacious? No, but I can always find a seat Was this provider willing to stay open late to accomodate your needs? Sometimes, but most of the time they don't

Does this provider treat every client equally? Definitely, they make everyone feel equally respected and cared for Was this provider's office too cold? No, it was fine Did this provider show attention to detail? Absolutely! They are always very careful and meticulous

Did you notice a foul odor when you arrived at this provider's office? No, I didn't notice an odor Was it easy to make an appointment with this provider? Yes! It was effortless! Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed Did they follow up with you after your appointment? Yes, they made sure I had everything I needed! Did you have to wait long after you arrived for your appointment? No, I didn't have to wait very long

Does this provider maintain a professional appearance? Yes, he/she looked very professional Was this provider's office easy to locate? Absolutely! It couldn't have been easier! Does this provider adhere to the customer is always right policy? Yes, they listen to all complaints and make every effort to correct the situation. Is this provider easy to reach in an emergency? Somewhat, they eventually called me back Did you spend a lot of time in the waiting room at this provider's office? I waited forever for my appointment!

Yes, it is very conveniently located Was this provider argumentative or easily angered? No, they were very calm and understanding Were the restrooms clean at this provider's office? Yes, I could tell they were cleaned regularly Do you feel that you could have received better service somewhere else? No, I don't think so
