This complaint is against Cityblock Health in Southfield, Michigan. I have been calling Cityblock six times since March about them sending my four consent forms to my house. I signed the consent forms during an online video call from my appointment with Dr. Tracey Alphin on March 17th. The appointment was concerning medications for my asthma.
