Was it clear that this provider takes pride in the appearance of his/her office? Yes, the office was well organized and taken care of Did this provider rush your appointment? No Does this provider always put the customer first? Yes, they always put customers first Were the staff members able to answer your questions? Yes, they were able to answer my questions Do you feel that you could have received better service somewhere else? No, I don't think so

Are you going to visit this provider again? Without a doubt! Does this provider always take that extra step to make you feel special? Absolutely, they always make me feel like I'm their only patient. Was this provider's office clean? Yes everything was neat and sterile Was this provider late to your appointments? No, they were on time

Was the waiting area spacious? Yes Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed Were you able to get answers to simple questions from this provider without scheduling an appointment? Yes Did this provider ever take personal phone calls while in the appointment room with you? He/she would never do that! Their full attention is always on me. Does this podiatrist use current and safe equipment? Yes; the equipment is very advanced!

No, they were very calm and understanding Was this provider's office too loud? No, it wasn't too loud Does this provider treat their staff well? Yes, they treat their staff with respect Did this provider ever dismiss your concerns as unimportant? No, they take every concern of mine seriously

No, I was always attended to Does this provider offer any coupons, discounts or promotions? Not that I have seen Were the staff members able to answer your questions?
