I was asked to sign an extraction waiver after my tooth was extracted Waiver explained possible... I was asked to sign an extraction waiver after my tooth was extracted Waiver explained possible consequences of procedure and some were alarming but I am not upset- tooth was cracked and needed to come out -IMHO it seems Dr Stock takes on so much he can't spend much time with patient
